First, you can write a draft and circulate if you do not have confidence. This is a better way than completely depending others.

Moreover, your request is vague. Are you working and seek permission ? or Are you a student and need permission?

Dear Ms. Poonam Gaur,

Thanks for your post. The matter you narrated is quite common and so, you need not to be embarrassed.
However, it would have been better and easier and more helpful for you had you opened a bit!!
Actually,the formats vary according to the variance of the purpose. As can well understand that out of your post, two types of assumptions can be made. First, that you want to get permission to do doctoral Research and second you want to conduct extramural Research funded by outside agencies and in each case the formats vary. If YOU are applying seeking permission, the format will be:
